Generally, the following documents are submitted for the purpose which acts as Identity and address proof:

Aadhar Card

Voter ID Card

PAN Card

Ration Card etc.

For date of birth proof documents like 10 class marks sheet

Nursing home/Hospital Report/Vaccination card

SSC from the recognized board by GOI

Certificate from School signed by Principal on School Letter Head can be used

An affidavit is also required from the applicant along with the application

It is important to first conduct a search at the concerned birth registrar’s office to verify indeed there is no record of birth available.

Once it is confirmed that the birth record is not available then an application along with supporting documents can be made at the concerned office.
